In retrospect, I wish someone had stood up on the first day of medical school and said, “Everything you will learn here has essentially two goals: how to diagnose disease and how to treat it. The goal is to teach you how to develop a differential diagnosis, how to evaluate a patient further to zero in on a diagnosis, and how to treat the disease diagnosed.” Although it seems like a simple statement, if it were truly understood by students at the outset of medical school, and if an effort were made during all pre-clinical teaching to explain how material is relevant to these goals, a strong framework for the learning, understanding, and application of medical knowledge would develop in each student from day one. The move toward integration of the preclinical sciences has been a large step toward this goal, and a much-welcomed change from the previously artificial separation of anatomy from physiology from pathology from biochemistry etc.

However, at present, there is no book that provides the student with a clear and concise overview of the mechanisms, diagnosis, and treatment of diseases from this integrated perspective. The goal of this book is to fill that perceived void, linking the study of pathophysiology with its relationship to normal physiology on the one hand, and with clinical reasoning in diagnosis and treatment on the other. The chapters may be read in any order and are linked via cross-references, where relevant. In the most prevalent curricular model at present, this book would ideally serve as a guide to the second year of medical school as well as a review for the USMLE Step 1. I essentially wrote it as the book I wish I had during second year to understand why I learned what I did in the first year, and why what I was learning in second year would be relevant to what followed. This book can help to serve as the “mental filing folders” for the deluge of information presented in the first two years of medical school. The book could also serve as a review for health professionals of any type at any level of training seeking a refresher on the basics. This book uses simple anatomical and physiological concepts to create a framework for the understanding of disease processes and the symptoms, signs, and lab findings that they produce. Each chapter begins with a brief discussion of “how an organ system works” (anatomy/biochemistry/physiology), which is then followed by what logically could cause malfunction of the organ system (pathology/pathophysiology). Following the establishment of this framework, specific disease mechanisms are discussed in this context, and the symptoms, signs, diagnosis, and treatment of these diseases are elaborated upon. In a complementary approach, the accompanying CD-ROM, designed by Dr. Stephen Goldberg, allows the student to proceed in the opposite direction: one can select a symptom, sign, or lab finding and see all of the many diseases that could cause it, classified by pathophysiological mechanism. we provide the free download, and if you want to buy the book you can visit amazon.
